  • all correspondence with any insurance company representative (including name of the person, employee #, call number, etc.)
  • medical documentation (ex. doctor reports, imaging results, lab results)
  • photographic evidence pertaining to the incident (such as scene of a car crash, damages incurred on the vehicles or to the road, images of any injuries, etc.) 
  • all  documents sent or received from any insurance company, including time and date
  • any claim process deadlines or filing due dates

The more thorough of a timeline and log you have, the lease wiggle room insurance companies have to instil doubt or deny a long term disability claim.
